Respondent(s) to the RFP shall provide services based on the Healthy Families America
services model throughout the Woodlake area, this includes Elderwood, Farmersville,
Ivahoe, Lemon Cove, Seville, Three Rivers, and Woodlake in Tulare County. These
services must be designed to provide pre-placement, preventative care to families who
are experiencing challenges that could place the children at risk of entering foster care.
The intervention aims to cultivate and strengthen nurturing parent-child relationships,
promote healthy childhood growth and development, enhance family functioning, and
afford children a safe, stable and supportive family environment by reducing risk and
building protective factors. Services shall conform to the Healthy Families America
model, be evidence-based, and well supported by the Title IV-E Prevention Services
Clearing House. Services shall include direct services and activities to both the child
(prenatal-5years) and the parent or primary caregiver of a child. Services shall be
provided with the approval of the referral/request to use FFPSA funding by the County
Child Welfare department. FFPSA funding shall not exceed twelve (12) months in most
circumstances. Healthy Families America is provided for a minimum of three (3) fiscal
years and up to five (5) fiscal years with the understanding that services are voluntary.
Services shall include but are not limited to home visitation, parent education and support,
information and referral services, and individual or group counseling.
