*The District shall award two (2) different CM/GC, one for each school.

One (1) original, and five (5)
hard copies, plus one (1) USB flash drive with an electronic version of all bid files in PDF, MS
Word and Excel as appropriate of the materials shall be submitted in a sealed envelope/box and
addressed to the Adams 12 Five Star Schools, 1500 East 128th Avenue, Thornton, CO 80241,
ATTN: Kelly Mickelson, Senior Buyer Purchasing Services. Each firm shall include its name,
address and the Adams 12 bid number on the proposal cover sheet along with the sealed
envelope/box. No fax submissions will be accepted. Proposals can be delivered by the USPS,
courier, by hand or other form of US Mail Delivery, including for example, Federal Express or
United Parcel Service.
