Summary of Specifications:
Communities That Care (CTC) is a structured process that supports a community to generate local responses and solutions to reduce substance abuse. A Facilitator/Coordinator helps guide a community’s CTC effort. Backed with extensive training and technical assistance from the Center for Communities That Care, as well as Boulder County Public Health (BCPH) program staff, specific health planning and data analysis deliverables are an integral part of the process. BCPH is seeking a contracted health planner to provide these services. BCPH is seeking a contracted health planner to provide these services during the first year of the project, which runs from July 1, 2016 through June 30, 2017. Available funds for this project shall not exceed $30,000
The CTC project is housed within the BCPH Community Substance Abuse Prevention (CSAP) Program. Working with the supervisor and other team members, the contracted health planner integrates data gathering, interpretation and presentation into a community planning process. The contracted health planner will also be available to respond to community questions about data and planning.
