24816C - Event Logging Implementation Software Services

The City and County of Denver, hereinafter referred to as the City, desires to solicit proposals relating to the purchase of Event Logging Implementation Software Services.
A.1 SUBMISSION OF PROPOSALS:
 
Your proposal shall consist of the following separate sections:
Section 1 

1. Signed Cover Sheet and Letter of Intent (Not to exceed 2 pages in length)
2. Resumes of Proposed Staff
3. Disclosure of legal and administrative proceedings and financial condition
4. Additional technical information in support of your proposal
5. Resumes and Account Team information (As specified in Exhibit A. - Scope of Work – Appendix H.)
6. Appendix H.  – Response Tab- 1--8

1. References and Vendor Information Forms
2. Greenprint Sustainability Information Sheet (if required)
3. W9 Form
4. XO101 Form
5. State of Colorado Certificate of Good Standing
