Summary of Specifications:
SCOPE: The work performed under this Contract includes the removal and replacement of existing concrete, new construction of miscellaneous concrete items and the installation of concrete items related to the paving of Alaska Avenue. These items include, but are not limited to, curb walk (Hollywood, Vertical, Mountable & Modified Type C),vertical curb and gutter, 6" thick flatwork, 10" thick flatwork, 10” thick crosspan, driveways, curb ramps, detectable warning surfaces, inlet work, asphalt patching, grading, and traffic control. The Contractor should anticipate some variation in the width of the curb walk items. Each Contractor is encouraged to verify the field curb walk width for the locations listed in Appendix A. The Contractor shall supply all labor, equipment, and materials necessary to complete the work in accordance with these specifications. Bid Condition: In addition to submitting a list of contract(s) on hand, the Contractor shall submit a list of all projects of similar magnitude and scope completed in the last three (3) years. Utilize ATTACHMENT B to document and submit these projects.The City of Longmont is seeking qualified Contractors with applicable experience comparable to the City of Longmont 2014 Concrete Rehabilitation Project. The Contractor must have satisfactorily completed at least three projects of similar scope in an established residential or commercial neighborhood within the last 5 years with each project valued at a minimum of $500,000. Utilize ATTACHMENT D to document and submit these projects. In addition, the Contractor shall be on the current CDOT list of prequalified contractors for work code 12.
