SCOPE: The work performed under this Contract includes the removal and replacement of existing concrete, new construction of miscellaneous concrete items and large scale restoration of irrigation and landscaping at street medians. These items include, but are not limited to, curb walk (Hollywood, Vertical, Mountable & Modified Type C), vertical curb and gutter, 6" thick flatwork, 10" thick flatwork, 10” thick crosspan, driveways, curb ramps, detectable warning surfaces, storm drainage inlet boxes, asphalt patching, grading and traffic control. The Contractor should anticipate some variation in the width of the curb walk items. Each Contractor is encouraged to verify the field curb walk width for the locations listed in Appendix A. Any curb walk section within +/- six (6) inches of the specified width will be paid at the bid unit cost. The Contractor shall supply all labor, equipment, and materials necessary to complete the work in accordance with these specifications.

BID CONDITIONS: The City of Longmont is seeking qualified Contractors with applicable experience
comparable to the City of Longmont 2018 Concrete Rehabilitation Project. The Contractor must have satisfactorily completed at least three (3) projects of similar scope in an established residential or commercial neighborhood within the last 5 years with each project valued at a minimum of $500,000. The projects must have included the construction of ADA compliant curb ramps. Utilize ATTACHMENT B & D of the Bid Submittal to document and submit these projects.
The Contractor must self perform a minimum of 60% of all project work. The Contractor shall self perform all forming, placing, and finishing of all concrete items on this project.
It is mandatory that all Bidders fully complete the Comparable Construction Project Experience form and provide all materials requested herein. Any Bidder that fails to comply with this requirement may be considered non-responsive.
