ADDITIONAL DOCUMENTATION AND FORMS
A. The bidder shall read and adhere to the "Required Contract Provisions,
Federal-Aid Construction Contracts" and the "Federal Labor Standards
Provisions" (Included in the Special Conditions). These documents are part of the
construction contract.
B. The Bidder is required to submit the "Certification Regarding Lobbying", Anti-
Collusion Affidavit" and the "Bidders List" (Included in the Special Conditions)
with the Bid documents.
C. The Bidder that is awarded the contract is required to submit, with the signed
contract, the "Socioeconomic Outreach Documentation Form" (Included in the
Special Conditions) for themselves and any subcontractor under them.
Davis Bacon Wages are required.
Railroad Specifications and Insurance also be required.
