El Paso County Department of Human Services (County) intends to award a sole source contract to Peak Vista Community Health Centers in the estimated not to exceed amount of $200,000.00 for Nurse Home Visitation that will serve to improve the health, safety, and well-being of children who are who are subject to a child abuse or neglect allegation and who are participating in programs provided by the Department through a partnership with a Medical entity who will provide families and Department caseworkers with medical support services. The Partnership will increase access to health care and community-based resources that support improved health outcomes for children and families through home visits conducted by Pediatric Nurse Practitioners.
