Tab 1 –Pricing and Schedule
Fill out pricing sheet (Attachment A). Indicate any exceptions to the scope of services with your proposed cost. Please provide schedule (separate from the pricing sheet) starting on July 1, 2022. All pricing must be final costs that include all taxes, shipping, etc. D49 is a tax-exempt entity. Tax exempt documents will be included in attachment B.
