2.1 Proposal Format
The Contractor shall prepare a proposal using the proposal form provided in Attachment
2 – Proposal Form. The Scope of Work Section of this RFP must be reviewed prior to
filling out the proposal form.
2.2 Proposal Submittal
The completed proposal form and a brief description of qualifications shall be enclosed
in a sealed package marked “Proposal for 2018 Mosquito Abatement Program” and
addressed as identified below. Each package shall be legibly marked in the upper lefthand
corner with the name and address of the Contractor.
Proposals will be accepted until 10:00 a.m., April 18, 2018. Proposals received after
this time and date will not be accepted and will be returned unopened. Submittals shall
be delivered to the following address (faxes will not be accepted):

The Town reserves the right to reject any or all Proposals, including without limitation,
the right to reject any or all nonconforming, nonresponsive, unbalanced or conditional
Proposals and to reject the proposal of any Contractor if the Town believes that it would
not be in the best interest of the Town to make an award to that Contractor, whether
because the proposal is not responsive, the Contractor is unqualified, and/or of doubtful
financial ability or fails to meet any other pertinent standard or criteria established by
the Town.
2.3 Fee Schedule
The contractor shall provide a lump sum bid price per the scope of work as identified in
section 3 (Base Bid). Additionally, the contractor shall provide a unit price for emergency
services involving adult mosquito control (Add Alternate).
