Description

The purpose of this Request for Proposal is to select an agency to perform billing & collection services for Castle Rock Fire and Rescue Department’s ambulance fees. The services shall require the successful Proposer (Contractor) to use the necessary forms for the billing and collecting of Ambulance fees from the following various types of payers:

Self-Pay (patient pays the bill)
All Commercial and Private Insurance, including Medicare Supplements & Replacements
Medicare
Medicaid
Workers’ Compensation
Personal Injury Protection/Medical Payments (Auto Insurance)

Four (4) copies of the Proposal are due by 5:00 p.m., on October 11, 2019, in a sealed envelope addressed to:

In the event a single proposal is received, the Town will, at its option, conduct a price and/or cost analysis of the proposal and negotiate the award, or reject the proposal and re-advertise. Comparing price quotations submitted on other quotations, current price lists, or other established or competitive prices would perform a price analysis.

The Proposer shall answer the specific questions on the following questionnaire (pages 10-12). This questionnaire is designed to determine the flexibility and the amount of services that can be provided. You may attach any examples that you wish with reference to the attachment on the “comments” section.
Note: The questionnaire MUST accompany all proposals

The Town of Castle Rock reserves the right to reject any or all proposals.
