Weld County is looking for a medical services company to provide on-site clinical medical services for primary care, acute medical conditions, initial and ongoing Workers' Compensation care, comprehensive Occupational Health care including fit-for-duty assessments, Medical Examinations for Commercial Driver Medical Certification per U.S. Department of Transportation regulations, and occupational and medical surveillance. Primary care services are to include lifestyle/disease management counseling and align with our safety and wellness program. Services shall include a package pharmaceutical component, acute care, and wellness interface to meet the needs of Weld County employees and dependents covered by Weld County health insurance.

Notice of Intent is REQUIRED to submit proposals. Due: 03/02/21 @ 5:00 PM (MST)

Proposals for the above stated merchandise, equipment, and/or services will be received until: Tues., 03/16/21 at 10:00 AM (MDT) (WELD COUNTY PURCHASING TIME CLOCK). Due to COVID-19, instead of an in-person bid opening, the submitted proposals will be read over a Microsoft Teams Conference Call on 03/16/21 at 10:30 AM (MDT). Conference call information is provided on page one of the bid specifications.

Proposal Delivery:
1. Due to COVID-19, one electronic copy of the proposal is required. Email proposals to bids@weldgov.com. If your proposal exceeds 25 MB please upload it to https://www.bidnetdirect.com. Only upload your bid to BidNet if your bid exceeds 25 MB. The maximum file size to upload to BidNet is 500 MB.
2. PDF format is required. Emailed bids must include the following statement on the email: "I hereby waive my right to a sealed bid". An email confirmation will be sent when we receive your proposal.
