Summary of Specifications:
SEALED STATEMENTS OF QUALIFICATIONS (SOQ) from qualified firms to provide design services for the Everitt Avenue Sidewalk Project shall be received by the BOARD OF COUNTY COMMISSIONERS OF BAY COUNTY, FLORIDA at the Purchasing Department, 840 W. 11th Street, Suite 2500, Panama City, Florida 32401 up until 1:30 PM (CST) on February 23, 2012. Sealed submittals shall be publically opened at 2:00 PM (CST) on February 23, 2012. Statements of Qualifications shall be submitted in a sealed envelope, plainly marked with respondent’s name, address, date and time of opening, and RFQ number 12-13 for Everitt Avenue Sidewalk Project. PRE-QUALIFICATION: Proposal packages must include proof of FDOT qualification (current FDOT Notice or certificate of Qualification) in the following categories. All design firms and their sub-consultants shall be qualified under Rule Chapter 14-75, Florida Administrative Code, in the following types of work: Group 3, Type 3.1 Minor Highway Design, Group 8, Type 8.2 Design, Right-of-Way and Construction Surveying, Group 9, Types 9.1 Soil Exploration, 9.2 Geotechnical Classification Lab Testing and 9.3 Highway Materials Testing.
Please submit one (1) original and eight (8) copies of your proposal package.
Description of Work: It is the intention of this RFQ to find a Proposer that is qualified to design and obtain all permits necessary to construct a sidewalk along Everitt Avenue from 11th Street to 15th Street (U.S. Highway 98). Plans are to be developed in general conformance with the FDOT Plans Preparation Manual (Latest Edition). The designer is to utilize the FDOT Standard Specifications for Road and Bridge Construction and FDOT Design Standards for Design, Construction, Maintenance and Utility Operation on the State Highway System to the maximum extent possible.
A MANDATORY Pre-submittal meeting will be held at 10:00 am (CST) on February 3, 2012 in the Public Works Department Conference Room at 840 W. 11th Street, Panama City, Florida 32401. Point of Contact will be Sheila Faries, Interim Purchasing Director, at (850) 248-8270. SOQs will only be accepted from firms in attendance at this pre-submittal meeting.
