Summary of Specifications:
The School District of Okaloosa County, Florida extends an invitation to all interested insurance companies, agencies, and agents licensed in the State of Florida to submit proposals for providing an Employee Dental Insurance Plan to approximately 3,939 eligible participants including retirees. This proposal, as well as the rates, will be guaranteed for at least two (2) years with an effective date of January 1, 2012. Optional quotes are also being sought with a guaranteed rate for three years. Currently the District pays for single dental coverage for their employees. We are looking for options pertaining to moving from full District paid to a voluntary employee paid plan, or any option in between. The proposal we are requesting is for two (2) or three (3) years beginning January 1, 2012. Continuation of the coverage is contingent upon annual review and agreement between the selected provider and the School District of Okaloosa County. Any renewal of the agreement will be subject to satisfactory servicing of the plan, satisfactory participation in the plan and acceptable continuation terms. The attached Dental Certificate is the plan now offered.
