RFP-14-05 - Voluntary Group Vision Plan

Summary of Specifications:
The School District of Okaloosa County, Florida extends an invitation to all interested insurance companies, agencies, and agents licensed in the State of Florida to submit proposals for providing a Voluntary Group Vision Plan to approximately 3,400 eligible participants. This proposal, as well as the rates, will be guaranteed for two (2) years with an effective date of January 1, 2014 with the option for additional renewal periods subject to mutual agreement by both parties. Any renewal of the agreement will be subject to satisfactory servicing of the plan, satisfactory participation in the plan and acceptable continuation terms. The attached Vision Certificates (2011 CERT - VISION HIGH and 2011 CERT - VISION LOW) describe the plan now available.
