The Santa Rosa County School Board is seeking pricing for Drug & Alcohol screenings and Physicals for the 2017-2018 School Year (July 1, 2017 through June 30, 2018). The screening and testing will consist of: -DOT drug screening and collection services including pre-employment, post incident, random and reasonable suspicion. Includes collection and MRO services. Need price for 5 panel and 10 panel screening. -Non-DOT drug screening and collection services including pre-employment, post incident and reasonable suspicion. Includes collection and MRO services. Need price for 5 panel and 10 panel screening. -Breath Alcohol screening services. -DOT Physicals. -Fitness for Duty Evaluations. -Locations and hours of operations that these services will be provided. -Type of medical provider performing these services (i.e. Nurse Practitioners, Physician Assistants, Physicians).
