Description

All prices must be all inclusive to include, but not limited to, product cost, shipping, inside delivery cost and complete installation of functioning equipment.

Successful bidder must provide and install listed equipment. Installation to include the following:

Removal and disposal of existing dishwasher including ventilation.
Preparation and modification of site to house dishwasher including, but not limited, to electrical circuit, capping of existing gas line, ventilation ductwork, ceiling or floor adjustments.
Delivery, uncrate and install new dishwasher.
Installation schedule to be mutually agreed upon between vendor and School District personnel.
Startup, checkout and demonstration by manufacturer.

Pot/Pan/Utensil Washer, front loading with split door design, (20) pan capacity, over/under rotating arms, 2/4/6 minute adjustable timer, up to (20) racks/hour, includes: (2) flat grids, (2) tray racks & (1) flat bottom rack, pre-rinse spray hose, sanitizing with Sense-A-temptm 700 booster rise, vent fan control, drain pump, stainless steel construction, 48v/60/3-ph, cULus, NSF, Energy Star.

Complete manufacturer’s literature, giving full description of the equipment being bid, must be submitted. Proposal price shall include inside delivery to the designated point according to the purchase order. Collect shipments will be refused.

Work Area: The contractor shall be responsible for cleaning up all work areas; free of debris, supplies and material. Contractor shall be responsible for the moving of all furnishings as necessary for access to any work area. Existing furniture/equipment is to be covered for protection during the installation process. The work shall be carried on in such a manner that there will be no interruption to or interference of classes. Work shall continue uninterrupted from the start date until the project is completed, except when instructed otherwise by School District personnel. The dumpsters located at the school site shall not be used for removal of debris and/or equipment.

License: The contractor will adhere to meet all Federal, State and local regulations and shall be licensed to do business in the County and Cities of Santa Rosa County as an active Certified Contractor. The contractor must maintain a current certified general contractor’s license with the State of Florida.

Tools, Materials, and Equipment: The Contractor shall be required to provide its personnel with all of the tools, power tools, trucks, and equipment necessary for the performance of their work.

Parking Policy: Contractor vehicles are not allowed to park on sidewalks. The vehicle operator shall be made aware that extreme caution shall be used to operate the vehicle in a way that will not be a hazard or hindrance to pedestrians using sidewalks.

Safety: The Contractor shall comply with the rules and regulations of OSHA and the Department of Labor. The Contractor shall be responsible for the safety, efficiency and adequacy of his plant, appliances, and methods, and for any damage which may result from their improper construction, maintenance or operation. The Contractor shall erect and properly maintain at all times, as required by the conditions and progress of the work, proper safeguards for the protection of workers and the public and shall post danger warnings against any hazards created by the construction operations.

References: The contractor is required to submit a list of three (3) references with their bid. These references must be from firms, organization and/or governmental agencies/entities for which services were performed satisfactorily in the previous five (5) years. Include the dates of services that were provided, client name, address and the name and phone number of the contact.

Event Note

After the Pre-Proposal Meeting we will conduct an on-site visit at Berryhill Elementary School Cafeteria and Pea Ridge Elementary School Cafeteria. Only those attending the conference will be allowed to submit a proposal. The first five (5) minutes of the conference will be for distribution of documents and for contractors to sign in. Contractors must arrive on time to be allowed to participate in the conference. Those arriving after the five (5) minute sign in period will not be allowed to participate or submit a proposal. The conference will be recorded, with the resulting DVD becoming a part of the documentation for this solicitation. Copies of the DVD will be made available to interested parties by notifying the purchasing office.
