Fulton County, Georgia (“County”) is seeking proposals for the provision of Aging Services on behalf of the Department of Senior Services (DSS).
Through the issuance of this Request for Proposal (“RFP” and/or “Proposals”), the County is soliciting Proposals from qualified Proposers for the provision of Case Management, Senior Center Management, and Volunteer Services in the three geographic service areas: South Fulton, Central Fulton, and North Fulton as outlined in Exhibit 1, Description of Geographic Service Areas and Locations. It is the intent of this solicitation to select one (1) or more vendors for each of the Geographic Service Areas, as identified in Exhibit 1. The successful offerors must have the capacity to provide all aspects of the services they are proposing to provide in the selected geographic service area. Offerors may present proposals for only one (1) Geographic Service Area. Following are service bundling options within the selected Geographic Service Area:
a. Case Management, Senior Center Management, and Volunteer Services
b. Senior Center Management and Case Management
c. Case Management and Volunteer Services
d. Case Management
Proposals will be separated by Geographic Service Area and be considered only for the service area indicated in the proposal.
