Fulton County, Georgia (“County”) is seeking proposals for the provision of Comprehensive Nutrition Care program services. Services include the preparation and delivery of meals, nutrition education, nutrition counseling, emergency meals, grocery baskets, temporary home delivered meals, temporary grocery cards, shelf stable meals, and special program or event support as described in this Request for Proposals. The successful proposers must have the capacity to provide services throughout Fulton County; however, the County reserves the right to award more than one contract.
Through the issuance of this Request for Proposal (“RFP” and/or “Proposals”), the County is soliciting Proposals from qualified Proposers for the Department of Senior Services Comprehensive Nutrition program.

Submitting Agency: Enter the name of the Submitting Agency as it appears in the Executive Summary.

The Pricing Form is attached as an Excel spreadsheet (Exhibit 6). The spreadsheet has formulas to spread the cost of Nutrition Education and Counseling across all meal types, based on the percentage of total meals for each meal type. The spreadsheet will also calculate the average cost of meal types. The proposer with the lowest average per meal rate will receive the full 10 points. For proposers with the second, third, fourth, etc., their average per meal rates will be divided into the lowest average per meal rate and multiplied by 10, the total points allowed for cost.
