Peoria County seeks proposals from qualified vendors to provide professional services for its Medicare Rehabilitation Program in accordance with the Conditions of Participation and such other regulations as are currently in effect and as are administered by the federal Center for Medicare and Medicaid Services (CMS). Heddington Oaks is a 214-bed skilled care facility which includes an 80-bed Alzheimer's unit.

Heddington Oaks is located at 2223 West Heading Ave., West Peoria, IL 61604. The services described in this RFP will be performed at this location.

MANDATORY PRE-BID
A mandatory pre-bid conference will be held on August 20, 2019 at 10:00 a.m. at Heddington Oaks, 2223 W. Heading Ave., West Peoria IL 61604. Tours of Heddington Oaks will be provided at this time, giving you the opportunity to familiarize yourself with the facility and ask any questions. Vendors may schedule appoints to come back and view the facility again; however, no questions can be asked during these follow-up visits. All questions shall be submitted in writing after the mandatory conference meeting.
