The purpose of this request is to secure quotations for services of a group health Third Party Administrator (TPA) on the County’s behalf. The County of Peoria Employee Health Benefit Plans are presently self-insured with Consociate as the current TPA, Unity Point Health as the primary PPO Network, MedTrak as the Pharmacy Benefit Manager, and US Preventive Medicine for wellness services. This RFP is for administrative services only.

MANDATORY PRE-BID MEETING
A mandatory pre-bid meeting will be held on March 20, 2019 at 1:00 pm CST @ Peoria County Courthouse, 324 Main ST., RM 402, Peoria IL, 61602. There is not a call-in option for this meeting.

RFP INFORMATION
Sealed bids – one original and two hard copies, as well as one complete copy in an electronic version – flash drive preferred – will be accepted by the County of Peoria, Purchasing Division, 324 Main Street, Room 501, Peoria, Illinois, 61602, until 10:00 a.m. on April 18, 2019. RFPs shall be submitted in a sealed envelope or package with the RFP name and number and the due date and time marked on the outside. Please include your company name and return address on the outside of the envelope. Please clearly mark any/all proprietary information, as defined in the Freedom of Information Act, in your electronic copy as such.
