Each bidder shall submit the original and one copy of the bid and supporting documentation in a sealed envelope no later than the time and date specified on the cover sheet. Bids received after the specified time shall be rejected and returned to the bidder unopened.

Each bid shall contain:

Unit prices and extensions.
A statement of the approximate number of personnel to be at the work site on a given work day, a list of equipment to be used on the project, and names, certification numbers, and length of certification of Certified Arborists.
1. References/ Experience documentation.
Include a copy of the Emerald Ash Borer Compliance Agreement
Include a copy of TCIA Accreditation if applicable
Include applicable insurance information supplied and financial forms completed.
Bid Bond or Check
Contact information and signature completed including an after-hours phone number other than an answering machine.
