RFQ-843 - Pressure Reducing Valve Annual Maintenance

Important Note to Vendors:
The City intends to award a Contract for one (1) year with an option to renew two (2) one (1) year annual extensions with both parties consent. The City estimates that the number of rebuilds identified below will be necessary in any given year. The City reserves the right to request additional rebuilds at the costs identified in this proposal in any given year as well as the right to reduce the number of rebuilds if deemed unnecessary by the City. The number of rebuilds identified shall be used to determine a base bid for this RFQ(see attachment). Provide the following on-line with you response: 1)On-line pricing 2)Three(3) references 3)Sample checklist to meet requirements of scope of work 4)Letter of commitment indicating response time of rebuild request. 5) Certification form with signatures
