Important Note to Vendors:
Uniforms are purchased on a fiscal year basis for approximately 30 employees. Employees are allowed to order shirts, jackets, shorts, pants and boots for a total not to exceed $300 per employee per fiscal year. Will need pricing to reflect sizes from Small - 2XL. Preferred company will have a brick & mortar store within 10 miles of Birmingham for the employees to visit for picking out clothes & fittings. If company has no brick and mortar store, will need to provide a representative to come to our location with clothing samples for trying on, catalogs, order forms and pricing. Time will need to be spent with each employee filling out the order forms. A few visits may be necessary if employees are not available. Company will need to provide invoicing detailing each employee and what was ordered with subtotals per employee. All clothing ordered will need to be separated and packaged per employee. All items will need to be delivered within 2 weeks of ordering. Please fill out the line items and also the attached word document with additional answers. This bid is for a 3 year contract, from July 1st 2021-June 30, 2024.
