Summary of Specifications:
On the behalf of the Ottawa County Department of Public Health (here and after referred to as OCDPH), the County of Ottawa (here and after referred to as the “County”) is seeking proposals for contract services to assist in the implementation of a three year pilot Pathways to Better Health, Community Health Worker Program throughout Ottawa County. This RFP shall serve as a tool to formalize negotiations and to enter into a contract with a chosen provider, if any. The chosen provider would be required to work closely with the OCDPH who is acting as the fiduciary of locally allocated resources and the Ottawa Pathways Steering Committee to guide and direct service delivery, quality assurance, evaluation, and sustainability planning
