Ottawa County, on behalf of the Department of Public Health (“OCDPH,” “Public Health”), is
soliciting proposals from experienced and qualified companies for a survey platform that provides
the department and its administrative end users the ability to self-manage surveys on behalf of the
OCDPH or County as a whole, a single division within Public Health, and/or a group or other
subdivision within the County’s organization. It is the County’s intent that the awarded vendor
will be the OCDPH’s primary online survey platform.
