(2) Fully Completed Appendix A-Price Sheet in Excel format. (3) Completed Ethics in Contracting Vendor Form and the Business Information Questionnaire. (4) Completed First Tier subcontractor Designation Form. (5) Completed Form W-9. (6) Submit your Fair Employment Practices (FEP) Certificate or a copy of your submitted application confirmation. (7) A copy of your Certificated of Insurance listing current coverages.
