1. The signed SFQ-2024 Quote Form. 2. The completed Appendix A - Price Sheet. 3. Review Appendix B - Contract, sign if you agree with the terms and conditions. 4. Complete the References List on Page 10 of the Solicitation for Quotes Form. 5. Complete the Ethics Form. 6. Complete the W-9 Form.
Questions are accepted online within this solicitation and via email to Michelle Brown at mbrown1@waynecounty.com until Thursday, April 18, 2024, 4:00PM EST. The answers to all questions will be posted via Addendum on Friday, April 19, 2024, by 4:30PM EST.
