Summary of Specifications:
Genesee County is seeking a CASE MANAGEMENT AND REFERRAL SERVICE PROVIDER to conduct a Program of coordinated, collaborative services with a focus on responsible use of limited resources, including a CASE MANAGEMENT component. Services are to be delivered to seniors aged 60 years and older, whose residences are located at scattered sites throughout the entirety of Genesee County's 639 square miles of land area. Genesee County welcomes innovative and unique strategies from proposers.
