RFP P21-99 - Disease Prevention and Health Promotion Services for the M.C. Department of H/S

The Morris County Office on Aging, Disabilities, and Community Programming is accepting proposals for Disease Prevention and Health Promotion Services, which are eligible under Title IIID-supported evidence-based programs (EBPs). Good health can support successful aging and independent living. Evidence-based programs empower older adults to take control of their health by maintaining a healthy lifestyle through increased self-efficacy and self-management. Proposals for services must provide services to Morris County Seniors age 60 years or older. Services will be funded from and in accordance with rules and regulations implementing Title III of the Older Americans Act of 1965 as amended. Grant award agreements will be for services provided from May 1, 2021 to December 31, 2021 and subject to the availability of funds and to the continuing satisfactory performance by the provider of service. The following Services are allowable for this funding opportunity: Physical Health Mental Health Physical Activity Education The goal of the proposal is to procure service providers capable of delivering the described services and activities while meeting or exceeding program standards and components outlined in the service taxonomy (attachment A). A list of eligible Evidence Based Programs are found in attachment B.
