CC22# 103EBP - Disease Prevention and Health Promotion Services for the M.C. Department of H/S

The Morris County Office on Aging, Disabilities, and Community Programming is accepting proposals for Disease Prevention and Health Promotion Services, which are eligible under Title IIID-supported evidence-based programs (EBPs). Good health can support successful aging and independent living. Evidence-based programs empower older adults to take control of their health by maintaining a healthy lifestyle through increased self-efficacy and self-management. Proposals for services must provide services to Morris County Seniors age 60 years or older. Services will be funded from and in accordance with rules and regulations implementing Title III of the Older Americans Act of 1965 as amended. Grant award agreements will be for services provided from January 1, 2022 to December 31, 2022. The County reserves the option to exercise up to two (2) twelve (12) month renewals at the discretion of the County subject to the availability of funds and to the continuing satisfactory performance by the provider.
