RFP-2016-065 - Development of an Outreach Program

Summary of Specifications:
In an effort to provide supports to individuals being discharged from State Psychiatric Centers/State Residential and in need of a residential placement, an RFP is being issued in the amount of $230,000, of which $15,000 must be allocated for service dollars. Providers will be required to develop an “Outreach Team” to assist in moving eight (8) individuals currently at the state PC (IP/State Residential) to community residential programs within one year. This enhanced staffing will target long-stay hospital patients (i.e., 1+ year) and will have, at a minimum, the ability to provide services to eight patients at all times. The successful proposer must be equipped to meet the June timeframe associated with this initiative by implementing enhanced staffing and services over and above that which is typically associated with licensed community residences and/or certified apartments.
