The County of Albany continues to see percentage of older individuals in our population increase with each decade. As the proportion of older persons grow, chronic disease and falls become more prevalent. As requesting by the Albany County Department for Aging, the County of Albany is seeking proposals for Highest Tier Evidence-Based Health Promotion, Disease Prevention and Recreational Programs, which can provide education along with activities that support healthy lifestyles, promote healthy behaviors and improve functional status for older adults in our community in the hopes of preventing and reducing chronic disease and falls.
