Summary of Specifications:
The County of Albany is seeking proposals for a Health Home Consultant as requested by Albany County Department of Mental Health. Due to the significant changes to the Health Home program. The Consultant will be responsible for completing required DOH and Lead Health Home tasks with regards to collecting, organizing and submitting necessary data, and completing required assessments which are essential components of an accurate and successful Medicaid billing process for Health Home Care Management Services. It is also expected that The Consultant will train staff in all of the new regulatory requirements in transition to Medicaid Managed Care.
