1.1 The County of Albany is seeking proposals for In Home Contact and Support Services for
older adults living in Albany County as requested by Albany County Department of Aging.
1.2 In Home Contact and Support Services for isolated and home-bound older adult living in
Albany County include:
a) Friendly Visiting: A scheduled visit to an older person to provide socialization,
recreation, and the opportunity to observe and report the client’s condition and
circumstances.
b) Shopping Assistance: assistance in obtaining food, medicine, or other necessities, whether
by shopping for the client or personal accompaniment of the client to the retail
establishments to provide basic essentials of daily living and the opportunity to observe
and report the clients’ condition and circumstances.
c) Telephone assurance counseling: (including automated systems)-Regularlyscheduled
telephone contact with an older person with follow-up as necessary and appropriate.
d) Supervision – Services provided in the home to monitor, guide and oversee the older
person’s actions and activities.
Other Services – Provided in the home to support the person including house cleaning,
laundry services, bill paying/other essential errands, items provided on loan such as
assistive devices
1.3 The County of Albany will implement a comprehensive program of services,
to be provided by one or more agencies.
