The Chautauqua County Department of Mental Hygiene herein after referenced as CCDMH is seeking request for proposals from Behavioral Health Practitioners in Psychiatry, Psychiatric Nurse Practitioner and Addiction Medicine. Practitioners will provide services in the OMH Licensed Article 31 clinics in Jamestown and/or Dunkirk and in the OASAS Licensed Article 32 clinics in Jamestown and/or Dunkirk and other programs operated by CCDMH located throughout Chautauqua County. These programs serve approximately 4000 residents per year including children/adolescents and adults. Diagnoses range from Adjustment Disorder to Bi-Polar and Schizophrenia. Practitioners will work with a comprehensive team of professionals including Social Workers, Counselors, Nurses, Peers, Family Support Partners and Care Coordinators. Practitioners will conduct psychiatric assessments, evaluate for medication, provide ongoing medication monitoring and prescribing, review ordered laboratory results, and review and sign treatments plans with staff. Practitioners will provide oversight to medical staff as appropriate and develop policies and procedures relative to psychiatric/addiction practices within the clinic. Practitioner will participate in Team Meetings as well as Incident Review and Quality Improvement Committees.
