Objective: Chautauqua County is seeking quotes for Employee Hearing Exams.
Contractor retains right to bid on any & all bid requests. Quotes accepted by the County may be used by any department or agency of Chautauqua County government, as needed, during the contract term.
Scope: Mobile on-site audiometric testing for employees at the following locations:
DPF 454 N. Work Street, Falconer NY 14733 (including South & Center Sewer District Employees)
2 Aero Drive, Dunkirk NY 14048
7934 Sherman-Stedman Road, Sherman NY 14781
Landfill 3889 Towerville Road, Jamestown NY 14701
In 2020, the total number of employees tested was 120. We are using that as the basis for the cost. Vendor must create and maintain a data base of past and present employee audiograms in accordance to 29 CFR 1910.95 and evaluation of audiograms and comparison with baseline audiogram. Also to provide on-site consultation and report review. Past history of audiograms can be provided by Chautauqua County if necessary in paper form.
Vendor is also required to provide employee training on:
a) Effect of noise on hearing
b) Purpose and value of wearing hearing protection
c) Advantages & disadvantages of hearing protection
d) Types of hearing protection including care, fitting, and use of each
Contract Term: The term of the contract will be January 1, 2021 to December 31, 2024
Contract: A sample agreement is attached for your reference. The County intends to enter into a contract consistent with its purchasing policy, usually with the submitter of the lowest quote.
Insurance: County insurance requirements are attached. Insurance must be in place prior to execution of the contract. Failure to provide properly completed insurance certificates within five days of the date of request may result in a declaration that your quote is non-responsive.
Quote: Please complete all items on the quote form. Failure to do so may cause your quote to be declared non-responsive, with the result that it will not be considered. Mileage or Travel charges must be included in the quoted price. Unless otherwise indicated, Chautauqua County will not pay any type of fuel surcharge. Please note that Chautauqua County is exempt from Federal and State sales taxes.
The County reserves the right to: extend the time period within which quotes may be submitted; reject all quotes; and/or reissue an RFQ, upon determining that doing so will increase the number of quotes or will otherwise be in the best interest of the County.
Payment: To ensure prompt payment, all invoices must reference the contract number and, as applicable, include a copy of the delivery receipt.
