The undersigned Bidder, having familiarized himself/herself with the specification and the Bid documents as prepared by the City of Albany Department of Water & Water Supply, and on file in the Office of the Board of Contract and Supply of the City of Albany, New York, hereby proposes to provide the goods in accordance with the Bid Documents and Technical Specifications.

The Total Bid listed on this sheet shall include all costs associated with the purchase of select herbicides and insecticides listed below. If applicable, shipping costs are to be included for delivery to City of Albany Department of Water & Water Supply, Alcove Reservoir, 156 County Route 111, Alcove, NY 12007. Please specify if local pick up is available.

AWB reserves the right to add, delete or modify the chemicals/products referenced below and may request additional products as necessary. The cost of such goods will be negotiated at that time.
