Title: School Physician Services
Proposal due date: May 29, 2024 until 2:00 p.m.
To All Providers:
The Connetquot Central School District is requesting proposals for the selection of School Physician Services. You are hereby invited to submit proposals in accordance with this Request for Proposal (RFP).
The District shall receive proposals on, or prior to 2:00 pm on May 29, 2024. Proposals received after stated date and time will not be considered. Late proposal will be returned or remain unopened in the District’s file, whichever is more convenient for the District.
Proposals shall be submitted in a sealed envelope addressed to the Connetquot Central School District, Purchasing Department, 780 Ocean Avenue, Bohemia, NY 11716 on or before the date and time as indicated above. The following information shall be clearly indicated on the face of the envelope:
• The name and address of the person or firm submitting the bid;
• The RFP number and name; including date and time;
The opening of this proposal will not be opened to the public. The names and/or recording of the proposing firms will be available through the Freedom of Information Law.
Proposals shall be irrevocable for a minimum period of forty-five (45) days from the date of proposal opening. Alterations to said proposals must be submitted in writing. Consideration shall be given only to those alterations, which may be caused by unforeseen circumstances beyond the control of the firm submitting said proposal. The Purchasing Agent or his/her designee, shall make such determination.
The Connetquot Central School District Board of Education reserves the right to reject any or all proposals that it considers not to be in the best interest of the school district or to accept part of any proposal.
Please read the attached material carefully before submitting your proposal. Incomplete proposals may not be considered.
