Summary of Specifications:
The Dutchess County Office of the Aging/NY Connects (DCOFA/NYConnects) intends to contract for Consumer Directed Home Care Services under Expanded In-Home Services for the Elderly Community Based Services Program. The contractor will provide services in conjunction with established DCOFA/NYConnects program policy and procedure. The purpose of this service is to support the consumer’s long term care needs and preferences as well as nursing home diversion and Medicaid spend–down prevention through the development of consumer directed service delivery.Consumers who are deemed eligible will recruit, interview, hire, train, supervise, schedule and terminate their employees.
