RFP-OFA03-15 - Health Promotion for Community Services for the Elderly (CSE)

Summary of Specifications:
The County of Orange (“County”) through its Office for the Aging is seeking a vendor to provide Health Promotion Services to eligible County residents. These services are funded by New York State through the Community Services for the Elderly Program (“CSE”). Multiple contracts may be awarded to ensure service coverage throughout the County. 

CSE was established to provide community-based, supportive services to frail, low-income individuals who are age 60 and over and need assistance to maintain their independence at home. The goals and objectives of CSE are as follows: • Enable the elderly to remain in their homes and participate in family and community life; • Reduce the reliance on institutional care; • Provide cost-effective, non-medical services; • Enable access to necessary information and support; and 

• Promote the continued involvement of informational caregivers.
