RFI-DOH01-12 (RFQ-DOH01-12) - RFQ FOR HOME CARE THERAPIES & PHYSICIAN CLINIC SERVICES

Summary of Specifications:
The Orange County Department of Health, Division of Nursing seeks qualified individuals and/or entities to perform: • evaluation, treatment, and therapy of its patients referred by the County’s Certified Home Health Agency, which includes Skilled Nursing Care and Therapy Services, including Physical Therapist, Occupational Therapists, Speech Therapists, Respiratory Therapist, Social Workers, and Registered Dieticians; and • evaluation and treatment of those patients seeking treatment at the County’s Tuberculosis and Sexually Transmitted Disease clinic, including Physicians and Nurses. All nurses and therapists performing home visits must have a minimum of three (3) years homecare experience. All nurses and physicians must have a minimum of three (3) years clinic experience. See attached Specification and Wage Table for therapy and physician specifications.
