1. The County, by and through its Department of Social Services ("OCDSS"), is seeking proposals from qualified Offerors for the provision of services to transition age youth in foster care. "Transition age youth in foster care" are defined as youth in foster care who either have, or are deemed to have, the permanency planning goal ("PPG") of another planned living arrangement with a permanency resource. As per section 430.12(f) of Title 18 of the New York Codes, Rules, and Regulations ("18 NYCRR 430.12(f)"), "a child deemed to have a goal of another planned living arrangement with a permanency resource means a child 16 years of age or older who resided in foster care for at least 12 months within the past 36 months and who has a goal of discharge to parents or relatives or a goal of adoption."
2. OCDSS is not designating a specific model program to be provided under a contract resulting from this RFP. Rather, OCDSS is looking to Offerors to propose programs that offer a wide array of services designed to meet the individual needs of, and provide support to, transition age youth in foster care. However, any proposed program must include services designed to prepare transition age youth for adulthood.
