RFP-OCDMH-13 - Supoorted Housing (Health Home, Long Term, Forensic Beds)

Summary of Specifications:
The Orange County Department of Mental Health (OCDMH) announces this Request for Proposals (RFP) for the development and operation of one or more of the following: Exhibit 1: 10 units of Supported Housing designated for Individuals with Serious Mental Illness as defined on Attachment A and who meet at least one of the following high need eligibility criteria: o Individuals with a serious mental illness identified as high users of Medicaid in need of supported housing who are referred by Health Homes. o Individuals with a serious mental illness who are residents of NYS OMH Psychiatric Centers or OMH-operated residential programs. o Individuals residing in Orange County who have a mental illness and who are high users of Medicaid Services. o Individuals with a serious mental illness who are being discharged from an Article 28 hospital or Article 31 hospital and in need of supported housing or for whom housing would assist in a hospital diversion. o There are no limits on the number of units (up to 10) an agency may request. Exhibit 2: 10 units of Supported Housing for persons residing in OMH psychiatric centers, Residential Treatment Facility (RTF), or OMH transitional housing programs serving Orange County residents. These are individuals with serious mental illness who have resided in an OMH psychiatric center, RTF, and/or OMH transitional housing program for one year or longer or for six months( at the discretion of the Single Point of Access (SPOA)) if no one meets the one year criteria. There are no limits on the number of units (up to 10) an agency may request. Exhibit 3: 4 units of Supported Housing for persons with serious mental illness being released from New York State (NYS) prisons and returning to Orange County. There are no limits on the number of units (up to 4) an agency may request.
