* Contract period is from January 1, 2021– December 31, 2021, renewable annually for up to a total of four (4) one (1) year renewal terms. Agencies awarded a contract as a result of this application and have satisfactorily complied with all requirements during the contract period will be offered the opportunity to extend the 2021 contract for each year up to a maximum total extension period of four (4) years.

Please note that it is anticipated that the New York State Department of Health (NYS DOH) will be assuming responsibility for this program in the future on a date to be determined. At that time, thirty (30) days written notice of contract termination will be given to providers.
