SSN-RC-2019-040 - Manage an Early Intervention and Pre-K Loan Program

The County of Rockland Health Department is considering the issuance of a Sole Source award to Jawonio to manage an Early Intervention and Pre-K Equipment Loan Program.
 
This award may be made on a sole source basis in accordance with the Rockland County Procurement Policy.
 
This agreement shall be for $6,000 to fulfill all requirements as described below.  An additional $200 maximum can be invoiced by the contractor, as needed during the agreement period to cover expenses related to transportation of large or bulky equipment from the contractor to families or from families to contractor.
 
The contract fills a need for:
 

• The contractor manages approximately 100 pieces of equipment and devices owned by the County and loaned to eligible county residents through the Early Intervention and Pre-K loan program.

• The contractor implements, utilizes, improves and maintains a data system to track the equipment and services for the Early Intervention and Pre-K Equipment Loan Program.

• Contractor will review appropriateness of the inventory of adaptive devices, matches the equipment and the child’s needs, follow-up with service providers and families regarding the loan of the equipment, including responsibility for the care of the equipment until it is returned to the lender in the same condition it was received and make repairs to the loan equipment as needed.

• Contractor shall train the recipient on the equipment provided for proper handling and storage of the equipment.

• Equipment shall be the sole responsibility of the contractor.

• Equipment Loans shall be for short periods of time, two-month period with the possibility of renewing for short term.

• A summary report shall be included with invoice, including loan program period date, number of students, number of new loans, number of renewals and number of returns.

• A log of all transactions shall be submitted to the County of Rockland Department of Health on a quarterly basis which include DOB, Parent/Guardian, Phone Numbers, Item Borrowed, Original Loan Date, Date of Renewal, Date of Return, Therapist Name, EI Program (Y/N) or Pre-K program (Y/N).

• Payments shall be paid on a quarterly basis.

• Transportation of Equipment shall only be billed for when equipment that has been delivered and/or picked up during the quarter and proof of delivery or pickup is required that includes parent signature.

• Experienced in providing Early Intervention Services

• Be a 501 (c)3
• Be governed by a Board of Directors
• Be up-to-date with all IRS filings
• Be up-to-date with all NYS filings
• Have a 2015 budget with Board approval
• Be fiscally sound
• Have a Non-discrimination Policy
• Have a Conflict of Interest Policy
• Maintain program records for the purpose of Early Intervention and Pre-K services and provide to the County all program-related information including proprietary materials, upon request
• Submit quarterly and year end reports, including log sheets for clients served
• Be available for on-site visits, announced and unannounced
