SSN-RC-2021-005 - Multi-Lingual Case Management Svcs to low-income residents - Konbit Neg Lakay

The County of Rockland is considering the issuance of a Sole Source award to Konbit Neg Lakay, (“the Provider”) for the provision of multi-lingual case management services to low-income residents of Rockland County. Funding amount is contingent on the adoption of the County of Rockland 2021 Annual Budget. As such, final funding amount may differ from the estimated amount.
TERM: January 1, 2021- December 31, 2021

This award may be made on a sole source basis in accordance with the Rockland County
Procurement Policy.

Program/Services shall include the following services:

1. Training and Education Services (three (3) programs as below)s:

• Immigration services  [Counted per applicant (eligible Rockland County resident), not per immigration service]
• Translation Services

3. Limited Intervention Services

• Information and Referral
• Assistance for parents communicating with school staff and advocating for their children
• Assistance with completing job applications
• Assistance with Application / Recertification process for Eligibility Programs

• Provider must ensure that its information, and all updates, are provided to and contained on the “2-1-1- database system” ; [2-1-1 is a program of United Way in the Hudson Valley]. Within thirty (30) days of contract execution the Provider will complete and submit to Hudson211 the “211 Agency Profile Form”, which can be accessed via the following link: www.hudson211.org/cms/contact.
• Provider will maintain and submit detailed records indicating all Performance Target Outcomes and contract requirements have been met throughout contract term to DSS upon request. Records must include a client identification number, name, initials, date and agency services delivered. Additionally, attendance records for all training must be provided. 

• Intake, case maintenance and management services** to help families achieve and sustain self-sufficiency
  • **for all federal and non-federal assistance programs; Medicaid, daycare; SNAP
• Child Preventive Services (referral, monitoring supportive and rehabilitative services provided to children under the age of 18 and their families to avert disruption of family unit)
• Child Welfare Services (services to protect and promote the welfare and safety of all children, including individuals with disabilities; homeless, dependent or neglected children)
• Family Preservation Services (services for children and families designed to prevent harm to children and help families at risk or in crisis)
• Family Support Services (community-based services to promote the well-being of children and families designed to increase the strength and stability of families and enhance child development)
• Staff Development Training and Educational programs (services to improve job performance and knowledge on social service programs)
• Refer and Inform Clients about the Home Energy Assistance Program (HEAP)
• For clients receiving employment services, Provider must include a written attestation that all of its clients through a contract with the Department of Social Services will be referred to the New York State's MyBenefits website, to ensure that all clients log on to complete an application and initial screening, informing clients about various employment supportive services and assistance programs, such as: Temporary Assistance for Needy Families (TANF); Federally Non-Participating Safety Net Employment Program (FNP); Supplemental Nutritional Assistance Employment and Training Program (SNAP E&T). 

• Demonstrate successful experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met.

• Must provide copy of current letter confirming recognition of Konbit Neg Lakay from the United States Department of Justice Board of Immigration Appeals
• Must provide copy of current letter confirming Konbit Neg Lakay’s Immigration Services are being provided by staff accredited by the United States Department of Justice Board of Immigration Appeals

• Must be located in Rockland County
• Must have multi-lingual (English, Spanish, Haitian-Creole) staff for these

• Must have established services location in Spring Valley, NY

• Be a 501 (c)3
• Be governed by a Board of Directors
• Be up-to-date with all IRS filings
• Be up-to-date with all NYS filings
• Have a 2021 budget with Board approval
• Be fiscally sound
• Have a Non-discrimination Policy
• Have a Conflict of Interest Policy
• *Provide a copy of agency’s most recently filed 990 IRS Form

• Maintain program records for the purpose of an audit and provide to the County all

• Submit quarterly and year-end reports, including but not limited to records that include a client identification number, name, initials, date and agency services delivered. Additionally, attendance records; sign-in sheets for any and all training or education program, including dates held; and class rosters; report that shows names on unduplicated individuals must be provided.

• Submit quarterly vouchers to RCDSS
• Quarterly Vouchers, which may only include expenses for the respective quarter, that are not supported by the proper documentation and/or not provided to RCDSS within thirty (30) days of the end of each quarter, or within 14 (fourteen days) of request by RCDSS, will not be considered for payment by RCDSS.
• Demonstrated Successful Experience providing the above services to eligible individuals

• Be available for on-site visits, announced and unannounced
