Description

The County of Rockland is considering the issuance of a Sole Source award to Rockland New Square Community Improvement Council, Inc. (“the Agency”) for the provision of Multi-Service Hub to residents of Rockland County. Funding amount is contingent on the adoption of the County of Rockland 2022 Annual Budget. As such, the final funding amount may differ from the estimated amount provided in this notice.

This award may be made on a sole source basis in accordance with the Rockland County Procurement Policy.

TERM: January 1, 2022 through December 31, 2022

Program/Services shall include the following services:

Social Services Resource & Referral Services
- Resource and Referral services are provided at 766 North Main Street, Spring Valley, New York.
- CIC will provide a minimum of 3000© unduplicated, low income* children, youth, families and senior residents (including those for whom English is a second language) of Rockland County** with opportunities to help them meet their needs for health, safety, social, moral and emotional growth by providing information and referral services regarding health, personal and family welfare, Housing-Section 8, community safety, facilitation of health insurance coverage, and application assistance, HEAP, SNAP, Medicaid and needs assessment.
  - Counselors will help residents navigate, understand and obtain current resources, benefits and services.
  - CIC will provide parents/caregivers with the knowledge and ability to access support services for their children.
- CIC will provide low-income* children, youth and families in Rockland County with strategies and services to feed the hungry, house the homeless and generally improve the health, welfare and safety of the community.
- CIC will professionally, responsibly, in a cultural/linguistic appropriate manner, meet the needs of the low-income population including seniors (a number of whom are fragile

elderly) that often are not able to successfully negotiate the maze of various government departments and agencies who provide needed services.

- CIC must ensure that its information, and all updates, are provided to and contained on the “2-1-1- database system” ; [2-1-1 is a program of United Way in the Hudson Valley]. Within thirty (30) days of contract execution the Provider will complete and submit to Hudson211 the “211 Agency Profile Form”, which can be accessed via the following link: www.hudson211.org/cms/contact.

Community Counseling Program Services
- Counseling Services are provided at 11 Adams Lane, Spring Valley, New York.
- Minimum number of individuals to receive counseling services is 150.
- CIC will offer professional counseling services, in a cultural/linguistic appropriate manner, to potentially and/or presently dysfunctional, low-income families, at-risk youth and socially/academically challenged children, including Dual Language Learners in Rockland County. A minimum of 3300© hours of counseling services will be provided to the individuals served.
  - Crisis intervention counseling will be available to low-income, bilingual, dysfunctional families
  - Children and Youth with service needs due to mental illness, developmental disabilities and/or substance abuse problems will have access to timely and appropriate services.
- Progress reports will be completed for each client after every counseling session and kept in confidential files.
- Intake forms will be completed for each client and will include the referring individual’s full name.
- CIC will provide low-income* Rockland residents with services to generally improve the health welfare and safety of the community.

If any of the above-referenced minimum required numbers are not met during the course of the contract term, the contract amount will be reduced proportionally.
* Client eligibility criteria ensuring service to verified eligible, low-income individuals in the specified communities; Low Income standard is met when individuals are recipients of public assistance or at or below 200% of the Federal Poverty guidelines. Low-income must be verified for individuals claimed under this contract and this verification must be included with supporting documentation. CIC will verify low income by requesting Medicaid or SNAP cards.

** Eligibility to establish residence, household income and size shall be verified by submission of necessary documentation. CIC will verify Rockland residency by requesting a utility bill and /or other document showing proof of address.

- Provider will maintain and submit detailed records indicating all Performance Target Outcomes and contract requirements have been met throughout contract term to DSS upon request
- 90% of participants will experience increased social/emotional/academic competency.
- CIC will maintain and provide copies of data and/or documentation that will contain at a minimum: log sheets for unduplicated clients served that specifies: date, time, name of client (including household members if services are provided to entire family), phone number, address, type of service(s) received and AC/CIN Case Number; CIC must provide

RCDSS with documentation that specifies the type of information and referral given (i.e.: application/consultation; Insurance, HEAP, SNAP; Needs Assessment specify if a senior citizen. If duplicates are identified, another qualifier (i.e. address, complete name, etc.) will be requested and must be provided to confirm that I&R has been given to unique individuals.

- CIC will maintain and provide copies of Project SHTEIG Monthly Report (for the Community Counseling Program Services) that contains at a minimum, the date the session held, Case ID Number, summary of Counseling session and total hours per case.

Demonstrated Successful Experience providing all the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met.

Minimum number to be served in 2022

If any of the above-referenced minimum required numbers are not met during the contract term, the contract amount will be reduced proportionally.

This notice is issued for informational purposes only and is NOT a request for competitive quotes.

Any non-profit agency that believes they are qualified to provide the services detailed above and is ready, willing and able to provide the services; shall respond to this notice no later than 3 PM on December 23, 2021. The response should provide specific details regarding the agency’s interest in providing such services as well as their prior experience in providing the same or similar services. In addition, the response should detail the agency’s connections to the Rockland County community.

Organizations submitting a proposal must meet the following requirements:

Provider must include a written attestation that it will be delivering more than one of the following services to all of its clients, as applicable, under the resulting contract:

- Intake, Case Maintenance and Management Services* to help families achieve and sustain self-sufficiency
  - *for all federal and non-federal assistance programs; Medicaid, daycare, SNAP
- Child Protective Services (referral, monitoring supportive and rehabilitative services provided to children under the age of 18 and their families to avert disruption of family unit)
- Child Welfare Services (services to protect and promote the welfare and safety of all children, including individuals with disabilities; homeless, dependent or neglected children)
- Family Preservation Services (services for children and families designed to prevent harm to children and help families at risk or in crisis)
- Family Support Services (community-based services to promote the well-being of children and families designed to increase the strength and stability of families and enhance child development)
- Staff Development Training and Educational Programs (services to improve job performance and knowledge on social service programs)

- Refer and Inform Clients about the Home Energy Assistance Program (HEAP)
- For clients receiving employment services, Provider must include a written attestation that all of its clients through a contract with the Department of Social Services will be referred to the New York State’s MyBenefits website, to ensure that all clients log on to complete an application and initial screening, informing clients about various employment supportive services and assistance programs, such as: Temporary Assistance for Needy Families (TANF); Federal Non-Participating Safety Net Employment Program (FNP); Supplemental Nutritional Assistance Employment and Training Program (SNAP E&T).

Provider shall meet the following requirements:

- - be located in Rockland County and serve all Rockland county communities
  - Must have multi-lingual (English, Hebrew and Yiddish ) staff for these programs.
  - All outreach materials must include the Rockland County logo.
  - Must be open and provide resource and referral services for a minimum of the following hours: Monday to Thursday: 9:00 am -5:00 pm; Friday: 9:00 am-12:00 pm. Counseling hours are by appointment only.
  - Be a 501(C)3
  - Be governed by a Board of Directors
  - Be up-to-date with all IRS filings
  - Be up-to-date with all NYS filings
  - Have a 2022 budget with Board approval
  - Be fiscally sound
  - Have a Non-discrimination Policy
  - Have a Conflict of Interest Policy
  - *Provide a copy of agency’s most recently filed 990 IRS Form

[*must be provided before any contract will be issued for 2022]

- - Submit quarterly and year-end reports, together with, but not limited to, the following data and/or documentation: log sheets for unduplicated clients served that specifies: date, time, name of client (including household members if services are provided to entire family), phone number, address, type of service(s) received and AC/CIN Case Number; CIC must provide RCDSS with documentation that specifies the type of information and referral given (i.e.: application/consultation; Insurance, HEAP, SNAP; Needs Assessment specify if a senior citizen. If duplicates are identified, another qualifier (i.e. address, complete name, etc.) will be requested and must be provided to confirm that I&R has been given to unique individuals.
  - Maintain program records for the purpose of an audit and provide to the County all program-related information including proprietary materials, upon request.
  - Be available for on-site visits, announced and unannounced.
  - Submit quarterly vouchers to RCDSS
  - Quarterly Vouchers, which may only include expenses for the respective quarter, that are not supported by the proper documentation and/or not provided to RCDSS within fourteen (14) days of the end of each quarter, or within five (5) days of request by RCDSS, will not be considered for payment by RCDSS.
  - Demonstrated successful experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met
