The County of Rockland is considering the issuance of a Sole Source award to Martin Luther King Multi-Purpose Center (“Service Provider”) for the provision of Multi-Hub Services to low-to- moderate income residents of Rockland County - $74,555. Funding amount is contingent on the adoption of the County of Rockland 2022 Annual Budget. As such, the final funding amount may differ from the estimated amount provided in this notice.
TERM: January 1, 2022- December 31, 2022
This award may be made on a sole source basis in accordance with the Rockland County Procurement Policy.
The Provider shall perform the following
Program/Services:
- After-School Program and Training Services
- After-School Program: Provide a minimum of 75* youth, ages 5 – 14, an after- school program that consists of enrichment activities, academic support programs for elementary and older youth, out of school recreational activities for youth after school and evenings.
- Training Services: Provide a minimum of 15* young adults, ages 14 – 21 years old, with training that includes but is not limited to:
- CPR / First Aid
- Creative response to conflict
- Fire safety
- Domestic Violence
- Employment Searches
- Education And Support Services
Provide a minimum of 100* adults, over the age of 21, with life and employment skills, parental education and support regarding educational techniques and rights of their children, obesity and nutrition education, and assistance locating and navigating available sources of aid for health, education, housing and other areas.
- Limited Intervention Services (Community Food Pantry Services and Information and Referral Services)
- Provide a minimum of 1,000* households with a Community Food Pantry, for food, clothing, furniture, electronics and other household items and information and referral services
- Information and Referral services include, but not limited to:
- Free access to computer, phone, and fax equipment
- Assistance with Application/Recertification process for Eligibility Programs
- Assistance with completing job applications
- Any client age 22 and over who visit the Service Provider for anything other than the food pantry must be referred to the New York State's MyBenefits website, to ensure that all clients log on to complete an application and initial screening, informing clients about various employment supportive services and assistance programs, such as: Temporary Assistance for Needy Families (TANF); Federally Non-Participating Safety Net Employment Program (FNP); Supplemental Nutritional Assistance Employment and Training Program (SNAP E&T).
- Minimum number of eligible clients to be served in 2022.

If any of the above-referenced minimum required numbers are not met during the course of the contract term, the contract amount will be reduced proportionally.
*Low-income and Rockland County resident eligibility is determined by client providing documents such as photo ID, proof of residency, birth certificate or custody documents for each child in the household, proof of social services assistance, and proof of income.
*Client eligibility criteria ensuring service to verified eligible, low-income individuals in the specified communities; Low Income standard is met when individuals are recipients of public assistance or at or below 200% of the Federal Poverty guidelines. Low-income must be verified for individuals claimed under this contract and this verification must be included with supporting documentation.

- Provider will refer and inform clients of “the 2-1-1 Helpline”, which is a free, confidential, multilingual service to assist the public in finding the answers to a variety of Health and Human Services related questions; it is open 365 days a year; and may be accessed by Dialing “2-1-1”, or by sending an email to 211info@hudson211.org or via their web site at: http://www.hudson211.org [2-1-1 is a program of United Way in the Hudson Valley].

- Provider will maintain and submit detailed records indicating all Performance Target Outcomes and contract requirements have been met throughout contract term to DSS upon request
Provider attests that it will be delivering one or more of the following services to all of its clients, as applicable, under the resulting contract:

- Intake, case maintenance and management services** to help families achieve and sustain self-sufficiency

- **for all federal and non-federal assistance programs; Medicaid, daycare; SNAP
- Child Preventive Services (referral, monitoring supportive and rehabilitative services provided to children under the age of 18 and their families to avert disruption of family unit)
- Child Welfare Services (services to protect and promote the welfare and safety of all children, including individuals with disabilities; homeless, dependent or neglected children)
- Family Preservation Services (services for children and families designed to prevent harm to children and help families at risk or in crisis)
- Family Support Services (community-based services to promote the well-being of children and families designed to increase the strength and stability of families and enhance child development)
- Staff Development Training and Educational programs (services to improve job performance and knowledge on social service programs)
- o Refer and Inform Clients about the Home Energy Assistance Program (HEAP)
- For clients receiving employment services, Provider must include a written attestation that all of its clients through a contract with the Department of Social Services will be referred to the New York State's MyBenefits website, to ensure that all clients log on to complete an application and initial screening, informing clients about various employment supportive services and assistance programs, such as: Temporary Assistance for Needy Families (TANF); Federally Non-Participating Safety Net Employment Program (FNP); Supplemental Nutritional Assistance Employment and Training Program (SNAP E&T).
- Demonstrate successful experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met.

Any non-profit agency that believes they are qualified to provide the services detailed above and is ready, willing and able to provide the services; shall respond to this notice
no later than 3 PM On January 20, 2022 (14 days after advertisement) of this announcement. The response should provide specific details regarding the agency’s interest in providing such services as well as their prior experience in providing the same or similar services. In addition, the response should detail the agency’s connections to the Rockland County community.

Organizations submitting an expression of interest must meet the following requirements:

- Must be located in Rockland County
- Must be accessible by public transportation
- Must have multi-lingual (English, French-Creole)staff for these programs.
- Submit detailed data to the Rockland County Department of Social Services on a quarterly basis for all units of service provided and for all program expenses.

- Regular attendance at Haverstraw, Spring Valley, Nyack, and Western Ramapo Collaborative meetings
- Be a 501 (c)3
- Be governed by a Board of Directors
- Be up-to-date with all IRS filings
- Be up-to-date with all NYS filings
- Have a 2022 budget with Board approval
- Be fiscally sound
- Have a Non-discrimination Policy
- Have a Conflict of Interest Policy
- *Provide a copy of agency’s most recently filed 990 IRS Form

- Maintain program records for the purpose of an audit and provide to the County all program-related information including proprietary materials, upon request
- Be available for announced on-site visits
- Submit quarterly vouchers to RCDSS
- Quarterly Vouchers, which may only include expenses for the respective quarter, that are not supported by the proper documentation and/or not provided to RCDSS within fourteen
(14)days of the end of each quarter, or within five(5) days of request by RCDSS, will not be considered for payment by RCDSS.
