The County of Rockland is considering the issuance of a Sole Source award to Community Outreach Center for the provision of Human Services to residents of Rockland County. Funding amount is contingent on the adoption of the County of Rockland 2021 Annual Budget. As such, final funding amount may differ from the estimated amount.
TERM: January 1, 2021 – December 31, 2021
This award may be made on a sole source basis in accordance with the Rockland County Procurement Policy.
Program/Services
Provide Human Services to a minimum of 4,295* total individuals (1,300 unduplicated [new] and 2,995 duplicated), as follows:
- Caseworker services that conduct intake evaluations for each individual to confirm residency and need for further assessment and/or assistance
- Services by appointment, telephone or walk-in to wide demographic including but not limited to disadvantaged and single-parent families, at-risk teens, senior citizens, unemployed, disabled persons, and individuals with limited English proficiency
- Comprehensive, culturally competent, and linguistically appropriate Case Management services to low income individuals and/or families who face language, cultural, financial and/or systemic barriers
- Provide client-centered support and guidance and make referrals as appropriate to prevent escalation of problems before they become complex, systemic and/or costly
- Maintain custom database that contains case records and tracks client information and activities including intake data, recovery plan, client compliance, referrals made, follow up, and case resolution
- Provider will refer clients, as appropriate, to the New York State's MyBenefits website, to inform them about various assistance programs and supportive services
- Familiarity with government programs and Rockland County resources including but not limited to:
- Public Assistance
- Health Insurance
- SNAP
- HEAP
- WIC
- Home care for the disabled
- Social Security
- Neighbor Fund
- Utility shutoff prevention; EmPower NY
- Child Care – Daycare
- S.S.I.
- Section 8/Public Housing
- Unemployment Benefits
- Child Health Plus
- Assistance to low income individuals by screening their eligibility for benefits and if necessary, assisting them to complete application forms
- Familiarity with Ultra- Orthodox and Hasidic Communities which are insular from media and internet from which to learn about government programs and other services in Rockland County
- Outreach through community newspaper currently reaching over 10,000 homes
- Demonstrated successful experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met
- Provider must include a written attestation that it will be delivering one or more of the following services to all of its clients, as applicable, under the resulting contract:
- Child Preventive Services (referral, monitoring supportive and rehabilitative services provided to children under the age of 18 and their families to avert disruption of family unit)
- Child Welfare Services (services to protect and promote the welfare and safety of all children, including individuals with disabilities; homeless, dependent or neglected children)
- Family Preservation Services (services for children and families designed to prevent harm to children and help families at risk or in crisis)
- Family Support Services (community-based services to promote the well-being of children and families designed to increase the strength and stability of families and enhance child development)
- Services of 2 full-time Caseworkers who are “Certified Navigators” by the New York State Department of Health
- Services of a part-time Caseworker who is a “Certified Assistor” by the New York State Department of Health
- Demonstrated successful experience providing the above services to eligible individuals and families
*Minimum unduplicated number to be served in 2021
If any of the above-referenced minimum required numbers are not met during the course of the contract term, the contract amount will be reduced proportionally.
* Eligibility to establish residence, household income and size shall be verified by submission of necessary documentation.
**Client eligibility criteria ensuring service to verified eligible, low-income individuals in the specified communities; Low Income standard is met when individuals are recipients of public assistance or at or below 200% of the Federal Poverty guidelines. Low-income must be verified for individuals claimed under this contract and this verification must be included with supporting documentation.
- Provider must ensure that its information, and all updates, are provided to and contained on the “2-1-1- database system” ; [2-1-1 is a program of United Way in the Hudson Valley]. Within thirty (30) days of contract execution the Provider will complete and submit to Hudson211 the “211 Agency Profile Form”, which can be accessed via the following link: www.hudson211.org/cms/contact.
